Please log into your umail and complete the rest of the form using the link found in your umail. The email will come from Docusign.
Students will receive a final signed copy of the document via your umail and again from Docusign. Please verify with your cohort leader any submission requirements they may have for proof of completion.
All students are reminded that they should be checking their umail every day as all University communications are sent to your umail. This includes forthcoming information regarding graduation, license application information and any other information related to the licensure year. It is expected that students are aware of the information made available to them via umail.
